Varicocele Grades: Understanding Your Diagnosis

Been told you have a “Grade 2” or “Grade 3” varicocele? Here’s what that grading system actually means for your treatment options.

If you’ve had a varicocele diagnosed, you may have heard your doctor mention a “grade.” This grading system helps guide treatment decisions, but it isn’t always explained clearly. Here’s a breakdown of what each grade means and how it factors into your care.

How Varicoceles Are Graded

Varicoceles are typically classified using a system based on physical exam findings, sometimes supplemented by ultrasound measurements of vein size:

• Subclinical: Not detectable by physical exam; only visible on ultrasound. Often found incidentally during a fertility workup.
• Grade 1 (small): Only palpable when the patient performs a Valsalva maneuver (bearing down, as if straining).
• Grade 2 (moderate): Palpable during a standard physical exam without needing to strain, but not visible to the eye.
• Grade 3 (large): Visible through the skin without any special maneuver, often described as looking like a “bag of worms.”

Does Grade Determine Whether You Need Treatment?

Not entirely. Grade is one factor among several — alongside symptoms, fertility concerns, and in younger patients, testicular size and development — that a specialist considers when discussing treatment. A Grade 1 varicocele causing significant pain may be a stronger case for treatment than an asymptomatic Grade 3 varicocele found incidentally. That said, larger varicoceles are generally more likely to be associated with symptoms and fertility impact, since a larger volume of pooled blood tends to correspond with more heat and pressure in the scrotum.

Grade and Fertility Concerns

For men being evaluated for infertility, higher-grade varicoceles are more commonly associated with abnormal semen parameters, though this isn’t a strict rule — some men with smaller varicoceles still experience meaningful fertility impact, and some with larger ones have normal semen analyses. This is part of why a full evaluation, including semen analysis when fertility is a concern, matters more than grade alone.

Does Grade Affect Which Treatment Is Recommended?

Varicocele embolization can generally address varicoceles across the grading spectrum, since the procedure works by closing off the faulty vein regardless of how large the resulting varicocele has become. Your specialist will use your grade, combined with your symptoms and goals, to help determine whether treatment makes sense and what to expect from it.
